What is the Health Homes Program?

Washington State's Health Homes Program is a Medicaid-funded initiative that provides intensive care coordination for individuals living with two or more chronic conditions or one serious mental illness.

Joining the Health Homes program is free for Medicaid members. Here's how it works:

Call us or use our referral form. We'll verify your Washington Apple Health enrollment and check if you qualify.

Meet Your Care Coordinator

We'll match you with a dedicated care coordinator who will get to know you, your conditions, and your goals.

Together we'll create your personalized HAP — a living document that guides every aspect of your care.
