Joining the Health Homes program is free for Medicaid members. Here's how it works:
Call us or use our referral form. We'll verify your Washington Apple Health enrollment and check if you qualify.
We'll match you with a dedicated care coordinator who will get to know you, your conditions, and your goals.
Together we'll create your personalized HAP — a living document that guides every aspect of your care.
We'll begin coordinating your care, connecting you to resources, and checking in regularly to keep you on track.

We work closely with hospitals, clinics, behavioral health providers, and community organizations across Washington State.
Online, phone, or fax referrals accepted. We follow up within 24 business hours.

We collaborate with your team, not replace it. HAPs are shared with consent.
